VACCINE/MASK MANDATE
I now require a proof of a vaccination record. If you are not vaccinated, I understand and respect your decision, but please be transparent with me. All that I can do is request that you to double mask. One of the two masks must be a surgical or N-95 mask. If you are vaccinated, I will still ask that you wear a mask. Please do not forget them. If you do not have one, one will be provided to you for an additional cost of $2.
YOU MUST SIGN THE LIABILITY WAIVER.
LIABILITY WAIVER FORM
This year, I will request all of my clients sign a liability waiver form prior to their appointment. I did not go to esthetician school, I studied Marketing and MIS at Temple University. Over the years, I've found that I have have a talent for certain services, which I conduct outside of my regular job. I don't have the time or money to go back to school. I apologize for any inconvenience and thank you for trusting me.
If you have any allergies, are using certain medications - oral or direct, have any diseases, are pregnant or are breastfeeding - contact me prior to booking or mention it in your comments.
For eyelash lifting/eyebrow lamination/eyebrow tinting more specifically, it is critical that I know prior to the appointment.
